KEY WORK RESPONSIBILITIES
Provides case management services to assigned caseload of residents.Prepares intake and admissions documentation for review with new residents in the Phoenix CRF Program or CRF Housing
Program prior to the resident’s move in date. Reviews Resident Handbook with new residents to ensure mutual understanding of, and agreement on, program guidelines and expectations.
Welcomes and orients incoming residents to CRF Program or CRF Housing Program. Helps residents with concrete needs around move in such as bedbug protocols, unpacking, getting familiar with the building, building routines regarding health and security and the resident community routine and location of laundry, neighborhood resources, staff locations and responsibilities.
Forms and maintains effective collaborative relationships of accountability on actions plans with residents for the purpose of facilitating resident’s success in a performance-based post-treatment transitional housing program in the areas of education, employment, recovery, volunteer participation and independent living.
Actively involves the resident in action planning and encourages the resident to take responsibility for the outcomes he or she is striving to achieve
Closely monitors and documents residents’ progress, maintains and updates client case management files through database system ensuring that monthly reporting deadlines are met.
Participate in food delivery and pick up.
Coordinates, links and brokers services matching residents’ needs.
Assists in liaising with treatment centers, support recovery programs, addictions and mental health workers, detox, shelters, primary care clinics, corrections, police, hospitals, outpatient clinics to reach prospective residents to improve continuity of care.
Assists in bridging residents to long-term un-serviced housing.
Maintain confidential (Protected “B”) resident documentation; sharing information only as it is required to ensure the safety of staff and other residents.
This is a full time position requiring availability to work Wednesday to Sunday from 11:00am to 7:00pm.
- Academic: Position requires a minimum of 2 year post-secondary education and training relevant to the criminal justice field or criminology line of education.
- Experience: Preference will be given to those with a minimum of 2 years’ experience in the criminal justice field within a community setting, assisting men and women affected by substance misuse, concurrent disorders, unemployment, lack of education, criminal justice involvement, homelessness and poverty related issues to become active participants in the social, economic and cultural life of the community. An equivalent combination of experience and education may be considered..
